Participant Agreement

SECTION 01

Participation Authorization

I am the parent or legal guardian of the participant named in this registration and authorize the participant to take part in soccer training, games, drills, conditioning, camps, clinics, scrimmages, and related activities organized by RIYADA F.C.

SECTION 02

Assumption of Risk

I understand that soccer and athletic activities involve inherent risks. These risks may include, but are not limited to, falls, collisions with participants or equipment, muscle strains, sprains, fractures, head injuries, concussions, dehydration, heat-related illness, and other injuries that may occur during athletic participation. I understand that these risks may exist even when reasonable safety precautions are taken. I voluntarily permit the participant to participate and knowingly assume the inherent risks associated with such participation.

SECTION 03

Participant Health

I certify that, to the best of my knowledge, the participant is physically capable of participating in RIYADA activities. I agree to disclose medical conditions, injuries, allergies, restrictions, or other information that RIYADA reasonably needs to know for safe participation.

SECTION 04

Emergency Medical Authorization

If I cannot be reached during a medical emergency, I authorize RIYADA F.C., its coaches, staff, or representatives to contact emergency medical services and obtain reasonable emergency medical assistance for the participant.

SECTION 05

Injury & Concussion Safety

I understand that head injuries and concussions may occur during soccer activities. If RIYADA staff reasonably suspect a concussion or other serious injury, the participant may be removed from activity. RIYADA may require appropriate medical clearance before the participant returns.

SECTION 06WORKING DRAFT

Liability Release

To the fullest extent permitted by Oregon law, I, on behalf of myself and the participant, release and hold harmless RIYADA F.C., its owners, directors, coaches, volunteers, employees, agents, and representatives from claims arising from the ordinary risks of participation in RIYADA F.C. activities and, where legally permissible, claims arising from ordinary negligence. This agreement is not intended to release or waive claims that cannot legally be waived, including claims arising from gross negligence, reckless conduct, or intentional misconduct.

Participant Conduct

RIYADA is committed to maintaining a respectful, safe, positive, and welcoming training environment. Players are expected to respect coaches, teammates, opponents, facilities, and equipment. Bullying, fighting, discriminatory behavior, harassment, repeated disruption, or intentionally unsafe conduct may result in removal from an activity.

Refund & Cancellation

The RIYADA Refund & Cancellation Policy is currently being finalized and will be presented before public registration launches.

SECTION 12

Monthly Membership

Your first $100 payment is due at signup. Your subscription renews automatically at $100 every month and includes four sessions per month until you cancel. There is no annual commitment. Cancel through Manage subscription before your next renewal; cancellation takes effect at the end of your paid period. The current month is not automatically refunded.
